At Minnesota Dignity Collaborative (MDC), we believe no one should have to face housing or health challenges alone. Through our Pathway to Home (PTH) Initiative, community volunteers — called Pathway to Home Navigators — walk beside neighbors experiencing housing instability, offering guidance, empathy, and connection to resources that can help.
