The Pathway to Home (PTH) Initiative is the cornerstone of the Minnesota Dignity Collaborative.
We train and support community volunteers accross the state — called Pathway to Home Navigators — to walk beside neighbors experiencing housing instability, helping them navigate housing, healthcare, and hope.
Our Navigators offer guidance, empathy, and encouragement every step of the way, creating meaningful human connections that help people move from crisis toward stability and belonging.
Together, we’re walking each other home.
Training: Volunteers receive guidance on how to offer compassionate, informed support and how to navigate local housing and healthcare systems.
Connection: Navigators listen, walk beside, and connect participants to the right resources and community supports.
Support: Through ongoing encouragement and practical help, volunteers remind people they’re not alone — and that a pathway home is possible.
Across Minnesota, more people are struggling to find stability as housing costs rise and safety nets fray. Pathway to Home was created to help fill the gaps.
